With the submittal/signature of this waiver the below named parties hereby waives, releases and holds harmless Chapman Basketball Academy, LLC - its directors, organizers, coaches, volunteers or designees from any and all claims, damages, costs, actions and causes of actions as the result of personal injuries sustained as a participant in the Chapman Basketball Academy Camps.
